Insurance Information

Musgrove Ear, Nose & Throat Ambulatory Surgery Center has contracts with the following Insurance Companies:

• Aetna
• CareFirst Blue Cross Blue Shield of Maryland
• CIGNA
• First Health
• Kaiser Permanente
• Medicare
• NCPPO
• PHCS
• Tri-Care
• United HealthCare/MAMSI

The physicians and the surgery center accept most major insurance plans. It is important to understand that your insurance company will be billed separately for the following professional services:

• Physician
• Facility
• Anesthesia (if indicated)
• Laboratory (if indicated)

Your insurance policy may require co-payments and/or deductibles that may apply to one or more of the services provided for your procedure. Please be aware that you may receive multiple ‘Explanation of Benefits’ and bills from each provider.

Insurance Companies will NOT guarantee payment when we call for authorization. You will be notified by our staff if the authorization is denied or if they specify that the procedure needs to be scheduled at a different location.

Each insurance policy provides different benefits. PLEASE call you insurance company for explanations specific to your individual policy.
