Self-Pay Information

An ESTIMATE FOR SURGICAL SERVICES will be mailed to you prior to your scheduled day of surgery. The estimate will include the fees for the SURGEON, the FACILITY, and the ANESTHESIOLOGIST (if applicable).

The anesthesia fees are based upon a time estimate. If your services take additional time the fees will change accordingly and will be billed by Safe Sedation, Inc.

Payment in full is required at least 2 weeks prior to the procedure date. If payment in full is not received the procedure will automatically be cancelled. If the patient cancels the procedure without giving a 48-hour notice or does not show up for the scheduled procedure, a deduction of 25% will be subtracted from the refunded amount. The fees are payable to the provider of the services resulting in making separated payments as noted above.
